Agree with above. I'd take it as someone just checking in and being friendly. So many people are not ok but look like they are it's good to check in on people from time to time in case you can help
I think you’re overthinking, it’s a really common phrase that people use when greeting someone and making small talk.
It depends on how this is being asked and their facial expression... if they look like scared or sad, then yes, I agree with you this may come accross a bit intimidating and makes you feel that something is wrong or you look awful. But 99% of the time staff members just ask if you are ok to see if they can help with anything, they are just being polite, sometimes you just look at them and make eye contact so they ask "are you ok there?" I don't mind at all if that's the case, it means they are being nice. It all depends on their tone of voice and expressions.
Meh I don't mind either way. It's quite a Britishism - my American mom still finds it a weird thing to say. Nice that someone is asking imho.
