It depends on what expenses you mean. Medicaid covers physical therapy in bundled appointments basically. So like, your primary doctor has to write a referral for blank amount of visits and Medicaid will approve blank amount of visits depending on what it is you’re needing it for.

We had wellloint and they paid for everything but as long as we had a referral

It depends on a couple factors Those being the state, the type of Medicaid/reason your on it(child, pregnant women, adult) and also the circumstances surrounding the need for the PT. If it’s for a child who has development delay they must score a certain rate based on an eval to get coverage, if it’s an accident based injury it’s usually covered. That goes the same for an adult, accident wise. Pregnant women needing PT. Is very debatable if they cover. This is in the state of Texas. I’ve dealt with Medicaid in a clinical setting and personally and it sucks both ways getting approval for things but I guess the same prolly can go for other insurances too.
