Maternity grant forms

You’ll need to print the form out and send your original maternity leave form Aswell that your midwife needs to sign off on. They send it back! And they usually confirm with you’re getting it on text

I did mine almost a year ago, but I can help you through it I think? Do you have your matB1 form from your midwife? If not you will need to ask for them to do you one.
