You can go straight to your hospital website too and find the section on subject access requests and request it that way x

How soon after giving birth are you requesting them? I’d also like to access mine but more for keep sake purposes!

Oooo I just did this, so I had to contact my hospitals patient advice liaison service (PALS) and they sent me a form for a subject access request for my health records - I’d assume this would be the same across country xxx
