Hey, my wife and I are also not pr yet (hopefully this year or next year though!). But my wife is from the UK so we have access to the interim Medicare card which pays for everything in hospital so we didn't need to pay anything for the birth of our daughter. What visa are you on atm? And which country are you guys from? Did you check if your country has an agreement with Australia? PR can take months, sometimes over a year depending on your situation, to be accepted so I would try and see if there's anything else you have access to.

You probably should ask your immigration agent that. Or call Medicare they'll be able to tell you, they were the one who told us what we were allowed since we've been here. I wouldn't be surprised if Hong Kong had an agreement or India since they're in the Commonwealth. You'll definitely have better, more accurate answers if you ask them directly.

Once you lodge your application and pay for it you can apply for Medicare. You will get interim card valid for one year or until your status is approved. You need to include your child into your application for him / her to get Medicare. I believe that's how it works.
