I’m also having to chase mine from my surgery having to wait for my community midwife to be in the surgery to do it she’s in tomorrow so hoping I can get it tomorrow 🤞🏻 I have just over 17 weeks left so want to make sure I get it to submit before i only have 15 weeks. Annoying having to chase it up tbf! 🙄

I’ve been told I will receive from midwife at my next appointment after 20 weeks (for me this is my 24 week appointment on 11th April?) I have heard people say you do have to ask sometimes?
