Fever and tired

So my baby(18mo) is acting super tired. Has 100.4 fever, feels super hot but her hands are cold. For the last week she's been skipping her nap but today she already took one and she's just laying on top of me right now, not really wanting to play. When she was playing with her blocks earlier she started breathing a little hard. Now she's breathing fast just laying on me as if shes tired. Should I take her to the doctor? She never likes to be held this much or sleep this much either.
