Tooth or Epstein Pearl?

Can anyone please tell me if this is normal? My baby boy is 3 months old and he has been showing lots of signs of teething, he is drooling constantly, chewing his dummy on one side of his mouth instead of sucking as normal, his cheeks are bright rosy red and flushed every day, he is in quite a lot of discomfort at the moment and is very grizzly throughout the day in comparison to his usual bubbly self, he has moments where he is screaming at the top of his lungs and sounds like he is in pain, he has developed the most angry looking nappy rash to the point his skin actually looks grazed and he is waking throughout the night 2/3 times but had been sleeping through 6/7 hours before. I’ve been giving him teething granules and this seems to relieve him slightly for a little while. When he was born the midwife thought he had a tooth but doctor said it wasn’t. He has this at the top right back side of his gums and this is the side he is tending to chew on. I’m not sure if it could be a tooth as usually bottom two front that appear first… or is it an Epstein pearl? (These are supposed to be painless which doesn’t seem to be the case for my little manny) Advice is very welcome as I’m feeling very sorry and helpless for my baby 😵‍💫
