Maternity grant

Ladies who’s done the maternity grant after their baby was born and what did you send as your proof? It just says a health professional has confirmed the birth, but what can you actually use for that? I don’t want to risk sending the wrong thing with it being so close to the 6 month mark and me leaving it until so last minute 🤦🏼‍♀️

I got my HV to write a letter for me ☺️

I just used a matb1 form and filled the unfilled bits in myself because both sides were signed by midwife x

I sent mat1b and one of the discharge form I had

Mat b 1 form. To fill the 20 page document out. It's on the gov website. Make sure it's got a signature and a date captured on the mat b form it needs to be the actual mat b 1 form. DWP question everything. Copy of baby's birth certificate is required. I was told I was getting maternity and then told on pay day I wasn't eligible so had to sort this out just after the baby was born. Just an fyi you won't be backdated 6 months. You will be backdated 3. So id get it in quick. Take your printed form and everything stated above to your local JCP and they will post it for you for free. Please note they cant check your forms. If you don't know what form I'm talking about let me know. X

It's Mat B only if your going through employer. If your going through DWP it's not. X
