Maternity grant

A midwife gave me a sure start maternity grant form with my matb1 form but me and my mum realised that it’s already filled out with some other random persons details ?❤️ What do I do cause I soon need to fill out the form so I csn get my grant I’m 27 weeks + 4 days 🤰🏽
