I requested for maternity certificates because I’m 21 weeks They sent me this and I was thinking the doctor or the midwife would already know what to write but since they asked me to send in information,I just don’t know exactly

If you have done this ,please let me know how you did it and they said I will be charged £20 Read this from the GP If you are requesting a private letter, we kindly ask you hand in a written letter with what you are requesting. Please be aware these letters have to be factual and based off your medical record meaning they may be rejected if necessary. The fee ranges, starting from £20 and this is paid in cash at reception where you will be given a receipt. Once we have your request, our admin team will action this accordingly.

Do you mean you requested a mat b1? That should be given to you at your 20 week scan if not midwife can give it to you at 25 week routine appointment

Yes I requested mat B1. I was sent this message

I was not given any on my scan day and I didn’t even ask cos I’m not aware I can get it there

I was told by my midwife I'd get one the next time I see her and I should be just over 20 weeks, I would message your midwife and ask her for it as your work till need it to work out roughly when you will be leaving for mat leave etc xx

I messaged my midwife directly and picked it up same day. (Had my 20 week scan last week)

Message your midwife not your doctor it’s a legal document that you require you do not pay for it, perhaps they think you’re after something else, but my midwife said hospital will give it if not she can get it x

You don't get given it at your scan, usually just at your next scheduled appt with the midwife, I think I was 24/25 weeks when the midwife did mine x

This is my second child so I won't see my midwife until 28 weeks. If I wanted the form sooner I had to ring to local maternity helpline after my 20 week scan, which I did today and they will post it out x

I was able to just walk into AnteNatal at my local hospital and get one issued in the spot with no charge x

I called my midwife after 20 weeks scan and they said they will send it by post

@Kirsty I’m having my 2nd baby too, but I have a 20 week midwife appointment? And a 24 week one

If it's the MATB1 you need the midwife will likely give this to you at your 24 week appointment

I'd message your midwife. I contacted mine and it's being posted to me. Free of charge x
