I had my red book for my little one when we were discharged and had our first health visitor appointment x
The red book is given by the health visitor usually before baby is born when they meet you for the first time to introduce themselves but if baby came early then it would be the first visit once discharged from nicu

We were given it when we got to nicu, then it was kept with her incubator until we were discharged. They should write in it for the first examination- done within 72 hours of birth, any hearing tests, vaccines which are done at 8 weeks (depending on how long you’re in for) and newborn hearing screening
When we were discharged from the hospital, we received it from the hospital staff.
