Birth plan

Does anyone have a birth plan template they can share? I didn’t make one with my first and kind of just went with the flow of labor. I tried a google search, but nothing I like popped up. I know the basics I want to include. This will be a planned c-section whereas my son was not planned (not emergency) and I was very unprepared for it. Just trying to prepare myself a little. Also any good resources for educating myself or preparing mentally? It’s coming up in less than 3 weeks and I thought I was good, but now I’m not so sure.
