I have to hand express only so how do I know I got all the milk? Having a pump doesn't work I only get less than an oz between the 2 but when I hand express I get more.

Depends on the settings on your pump.

I've learned that the harder the sucking pressure doesn't truly mean the better it gets the milk out! I'm currently pumping exclusively for my third baby right noqaaaw and it's the gentler suction that feels like breastfeeding works the best and try not the squish the cups against your boobs because I've subconsciously done that and just end up with soreness.

Make sure you’re using the right sized flanges. Get measured if you’re unsure. I have an Elvie hands free and hate it. I also have a medela pump in style, it’s great or really any medela. I loved manual pumps the most though and responded well to them.

I'm size 19 but only have 24 and the lactation nurse said it would be ok but I literally can't use a pump and my boobs are getting really dry from me squeezing on them

Would you be willing to buy the inserts for the flange? They make a huge difference and prevent nipple cracking. I don’t trust lactation consultants. They aren’t well educated on pumping, just breastfeeding. There are a lot of cheat sheets on pump settings online.
